Instructions for Teletherapy Services

1.) Please visit the Client Forms tab on this website and click on the button named Virtual Visit Informed Consent. Then read, print out and sign the form. You can either scan and email it to me or text or email me a photograph of your signed Virtual Therapy Consent Form. I must have this form signed and sent to me before we can begin our teletherapy session. 

If you are a new client, you will also need to fill out the Client Intake Packet and email those to me along with a photo of your insurance card. 

2.) You will be sent an appointment reminder and a personal HIPAA compliant link via text or email the day before your appointment.

3.) Once you receive the link and it is time for your appointment, you will click on the link from your smart phone or computer. You will be prompted to check into the virtual waiting room by entering your name and enabling your camera and microphone on your device. This will allow us to see and hear each other when the appointment begins. You can also visit prior to your appointment to become familiar with how your device will work and become familiar with the platform before your scheduled appointment.  

4.) As your provider, I will see your name appear in the waiting room list when you sign in. No one else will be able to see your name, only me. Once I click on your name our appointment will begin. 

5.) Your appointment will last approximately 45-60 minutes. 

6.) Before ending our call we will schedule and confirm your next appointment. 

7.) After your appointment, you can make a co-payment by visiting the secure payment link at the top of this website.
